There is a statement of consent for the non-appearing parent that will need to be completed and notarized.  You would need to bring this form and a front and back copy of the non-appearing parent's drivers license with you when you apply.  The statement of consent can be found online at https://travel.state.gov/content/travel/en/passports.html .
