You can obtain an application for a passport in our customer service department or online at https://travel.state.gov/content/travel/en/passports.html .  If it is the initial issuance of the passport, you will file your application in our customer service department.  If it is a renewal, you would mail in the application and the address is located on page 3 of the application.
