If a death certificate has been recorded in the official court records, you can request a copy via email clerkrecords@putnam-fl.gov, make request at https://putnamclerkfl.justfoia.com/publicportal/home/track or in person in our customer service department.  NOTE:  Social security numbers and any health information will be redacted.  The fee for a copy is $1 per page and $2 for certification, if requested.
