The payment plan agreement request form can be found on our website, https://putnamclerk.com/forms/ .  Completed form must be turned in to our Customer Service Department.  There is a $25/case fee plus a $10 minimum down payment to initiate a payment plan.  You can do a payment plan on past due fines.
