The clerk can postpone you once for up to six months.  If you would like to request to be extended or have your jury service rescheduled to a later date, then you may request such an action in writing by providing a doctor's letter (should your reasoning be medically related) and/or a personal letter explaining your situation by submitting such documentation via fax to 386-329-0330, or mail to PO Box 758, Attn: Jury Management, Palatka, FL 32178-0758, or email to jury@putnam-fl.gov.
